岐阜胸部外科フォーラムでの講演

明日岐阜で当センターの左開胸弓部大動脈人工血管置換術124例について講演させてもらいます。この手術は大網充填後や複雑な冠動脈バイパス術後など、正中切開が難しい場合や弓部下行の広範囲が急速拡大している場合、または感染などに有効な術式で、正中切開に取って代わるものではありません。そのような症例なので、緊急、準緊急が半分近くを占めておりましたが30日死亡はゼロ、在院死亡は1.6%でした。この手術を標準化することで複雑な動脈瘤に対してのより安全なアプローチ方法が増えました。

Tomorrow in Gifu, I will be giving a lecture on 124 cases of left open aortic arch graft replacement at our center. This surgery is effective in cases where midline incision is difficult, such as after omental filling or complex coronary artery bypass surgery, when the descending arch is rapidly expanding over a wide area, or when there is infection, and is not intended to replace midline incision. Because of these cases, nearly half were emergency or semi-emergency, but there were no 30-day deaths and 1.6% in-hospital deaths. Standardizing this surgery has increased the number of safer approaches to complex aneurysms.

尾﨑医師の現地レポート①inタイ

現在、タイにて短期の手術指導を行っている尾﨑医師の現地活動も今後お伝えしていきます！

今朝、Siriraj病院全職員向けの朝カンファレンスで自己紹介をしました。
司会者からの最初の紹介が「Kawasaki Aortic Center is one of best aortic treatment center in Japan and also in the world.」でした。
とても誇らしいことです。

写真はChiengmai大学の朝です。今日から出張手術です。

川崎大動脈センターでは、2024年12月から新たな試みとして当センター医師を年単位でタイに派遣しKAS: Kawasaki Aortic Surgeryを普及するprojectを開始しています。

静岡県から川崎（神奈川県）へ～180kmの救急搬送～

本日、静岡県から急性A型大動脈解離の患者さんをヘリコプターで受け入れる予定でしたが、強風のためヘリの運航が中止となりました。そのため、現在は約180km離れた川崎の当センターまで、救急車で搬送されています。

搬送時間を短縮するために、当院のドクターカーが途中まで出向き、救急車と合流する体制をとっています。これは、患者さんができるだけ早く、そして安全に当院へ到着できるようにするための取り組みです。

We were scheduled to receive a patient with acute type A aortic dissection from Shizuoka Prefecture via helicopter today. However, due to strong winds, the helicopter could not fly. The patient is now being transported by ambulance across a distance of approximately 180 km to our center in Kawasaki.

To shorten transport time, our doctor car meets the ambulance halfway—a system we use to ensure patients reach us as quickly and safely as possible.

501例の経験を胸に ― 川崎での2年間

フィリピンから来られたポチョロ先生は、川崎での2年間の大動脈外科フェローシップを終え、無事に帰国されました。この2年間で、ポチョロ先生は501件の大動脈手術のアシスタントを務め、幅広い経験を積まれました。台湾のリン先生も同様に1ヶ月のオブザーバーシップを終え、明日帰国されます。 送別会は、フィリピンからの麻酔科研修生や台湾からの多くの訪問医師の方々にもご参加いただき、大変活気に満ちたものとなりました。皆様の情熱がアジア全域の大動脈医療の発展を牽引していく姿を、心から楽しみにしています。

Dr. Pocholo, who came to us from the Philippines, has safely returned home after completing his two-year aortic-surgery fellowship in Kawasaki. Over those two years he assisted in 501 aortic operations and soaked up an extraordinary range of experience. Dr. Lin from Taiwan has likewise finished his one-month observership and will depart tomorrow. Our farewell gathering was wonderfully spirited, joined by anesthesiology trainees from the Philippines and many visiting physicians from Taiwan. I look forward—with genuine excitement—to seeing their passion drive the growth of aortic care throughout Asia.

Aortic Asia 2025に参加

Aortic Asia 2025に参加する機会に恵まれ、KACの経験に基づいた急性スタンフォードA型大動脈解離の治療戦略、および胸腹部大動脈瘤治療における一期的アプローチと段階的アプローチの検討について発表しました。会議は非常に有意義なものとなり、質疑応答では活発な議論が交わされました。 会議主催者のWorawong先生をはじめ、タイの大動脈外科医、この分野を牽引する日本の第一線の専門家、そして世界中の優れた医師の方々など、著名な同僚の方々と交流する機会を得られたことは、本当に素晴らしいことでした。特に、以前X（旧Twitter）で繋がっていたWei先生と、ついに直接お会いできたことは大変光栄でした。 この会議は、世界中の大動脈外科コミュニティの経験と学びを共有するための貴重な場となりました。有意義な交流に感謝するとともに、これらの尊敬すべき同僚の方々と、この分野の発展に向けて引き続き協力していくことを楽しみにしています。

I had the privilege of participating in Aortic Asia 2025, where I presented on treatment strategies for acute Stanford Type A aortic dissection based on KAC’s experience, as well as considerations regarding single-stage versus staged approaches in thoracoabdominal aortic aneurysm treatment. The conference was exceptionally engaging, with lively and active discussions during the question-and-answer sessions. It was truly wonderful to have the opportunity to interact with such distinguished colleagues, including Dr. Worawong, the conference organizer, Thai aortic surgeons, leading Japanese experts in the field, and outstanding physicians from around the world. I particularly enjoyed finally meeting Dr. Wei in person, with whom I had previously connected on X (formerly Twitter). The conference provided an invaluable platform for sharing experiences and learning from the global aortic surgery community. I am grateful for the meaningful exchanges and look forward to continued collaboration with these esteemed colleagues in advancing our field.

ジェネラルホスピタル（フィリピン）からのゲストを迎えて

フィリピンジェネラルホスピタルよりたくさんのゲストが見学に来てくれました。現在、麻酔科と人工心肺技師のところにも2人見学に来られており、合計8名の方々が来てくれています。見学していただく手術ですが、本日は左開胸が2件、腹部大動脈が1件です。

Many guests from the Philippine General Hospital came to observe. Currently, two guests from the anesthesiology department and the cardiopulmonary bypass technician department are also visiting, for a total of eight guests. The surgeries they will be observing today are two left thoracotomy surgeries and one abdominal aortic surgery.

【4月の主な手術実績】

川崎大動脈センターにて、4月の胸部大動脈手術は52件でした。

主な内訳は以下の通りです。
・ARR（大動脈基部置換術）：11例
・TAR（弓部大動脈全置換術）：17例（うち左開胸アプローチ3例）
・TAAAR（胸腹部大動脈置換術）：11例
・ステントグラフト抜去術：3例
内容としては85歳以上の方の胸腹部大動脈瘤III型手術、透析患者の胸腹部IV型、David後のAVR+HAR、re-do ARR+大網充填、80歳以上の左開胸ステント抜去、110キロ超えの循環停止下胸腹部大動脈瘤II型+ 両側内外腸骨動脈再建術などでした。緊急手術は比較的少なく13件でした。

フォローされている病院で治療できないと言われて自分たちで調べて当センターに来られた方も多数おられます。 大動脈瘤、解離に関しての紹介やセカンドオピニオンはいつでも受け付けております。また当センターからセカンドオピニオンで紹介した方もいらっしゃいます。セカンドオピニオンを受けることは患者さんの権利です。

In April, we performed 52 thoracic aortic surgeries. The major procedures were:
•ARR (Aortic Root Replacement): 11 cases
•TAR (Total Arch Replacement): 17 cases (incl. 3 via left thoracotomy)
•TAAAR (Thoracoabdominal Aortic Replacement): 11 cases
•Stent graft explantation: 3 cases
Notable cases included: – Type III TAAAR in patients over 85 – Type IV TAAAR in a dialysis patient – AVR + HAR after a previous David procedure – Redo ARR with omental flap coverage – Stent explantation via left thoracotomy in a patient over 80 – Type II TAAAR under circulatory arrest in a 110 kg patient with bilateral iliac reconstruction Emergency surgeries totaled 13, relatively few for the month.

Many patients came to us after being told treatment was not possible at other hospitals they were initially referred to. We always welcome referrals and second opinions for aortic aneurysms and dissections. Likewise, we sometimes refer patients from our center for second opinions elsewhere. Seeking a second opinion is every patient’s right—and it may open the door to life-saving treatment.

1ヶ月にわたる当センターでの研修生

台湾から2025年4月1日ー4月28日 Dr.Heng Suが当センターで見学を行いました。数えてみると1ヶ月で10件の胸部大動脈手術に参加し、20件程度の手術見学をしてもらえました。帰国してから彼は新しい施設のセンター長になるということで、これからも協力関係を続けていきます。

From April 1-28, 2025, Dr. Heng Su from Taiwan visited our center. In just one month, he joined 10 thoracic aortic surgeries and observed ~20 procedures. As he returns to lead a new facility, we’re excited to continue our collaboration!

多国籍チームと共に、新しい白衣で新スタート

今日は新しい白衣が届いたのでみんなで写真を撮りました。インド、イタリア、フィリピン、台湾の先生方が一緒に写っています。また新たに加わったメンバーと今年から大動脈外科医を目指す先生、大動脈外科に進もうとしている学生もいます。この写真がまた一つ新しい想い出になりました。

A new white coat arrived today, so we all took pictures. Surgeons from India, Italy, the Philippines, and Taiwan are photographed together. In addition, there are new members, doctors who are aiming to be aortic surgeons from this year, and students who are trying to advance to aortic surgery. This photo became another new memory.
