川崎大動脈センターでは今朝まで緊急Bentall手術、その後胸腹部アプローチのEVAR後AAARを行い、緊急弓部置換が今行われております。そしてERにはB型解離が運ばれてきました。さらにA型解離とEVAR後仮性動脈瘤の患者が運ばれてきます。また冬が来たなという感じです。血圧管理には気をつけて下さい。
At the Kawasaki Aortic Center, we performed emergency Bentall surgery until this morning, then EVAR followed by AAAR using a thoracoabdominal approach, and emergency arch replacement is currently being performed. 急に寒くなってきています。そのためか今日はA型解離が4件。2件は他の病院からの紹介でしたが、2件は直接当院の救急外来に来た方でした。状態が悪くてもオペ室に連れて行けば何とかなる。そう思える1日でした。一緒に頑張ってくれるER、技師、看護師、麻酔科、ICUスタッフに感謝です。

ステントグラフト後下行大動脈瘤拡大。最近増加している疾患ですが、私達は追加ステントを考慮することはせず左開胸弓部下行置換術を行いステントグラフトも置換範囲は抜去しています。ステントグラフトを繰り返し挿入されると開胸手術はより難しくなるため、早めに人工血管置換術に踏み切る方が安全です。
