本日は静岡県から心臓血管外科医を志望する研修医1年目の先生が見学に来てくれました。腹部大動脈置換、胸腹部大動脈人工血管置換、急性大動脈解離に対するBentall手術+部分弓部置換術を見学してもらいました。彼を見ていると14年前の山本先生の手術を初めて見学した日を思い出しました。
A first-year resident aspiring to be a cardiovascular surgeon from Shizuoka visited for observation today. He saw AAAR, TAAAR, and a Bentall+HAR. Watching him reminded me of the day 14 years ago when I first observed Dr. Yamamoto’s surgery.
今月の大動脈手術件数。私は13件。その他のスタッフたちはそれぞれ11件、10件、8件、3件、1件と続きます。緊急はいつでも対応するために皆が手術を行える体制を整えています。スーパードクターが1人いるよりも手術をきちんと出来る職人が多数いるチームを目指すのがKACです。
This month, I performed 13 aortic surgeries. Other staff completed 11, 10, 8, 3, and 1 each. We’re ready for emergencies with a team skilled in surgery. Kawasaki Aortic Center values a team of proficient surgeons over having just one super doctor.
夜間緊急手術後、何故か目が醒めてしまい、眠らなくても大丈夫かと思ってしまうことがある。しかし月に20件以上の緊急手術がある状況では休まないでいると次の手術に対応できない。手術が終わったらまず寝る。起きている医者がいたらまず寝かせる。10月は20件で済んだが、今月はそれを上回るペース。
After emergency surgery at night, I sometimes wake up for some reason and wonder if I don’t need to sleep. However, in a situation where there are more than 20 emergency surgeries per month, if they do not take time off, they will not be able to prepare for the next surgery. After the surgery, I will go to bed first. If any doctors are awake, let them sleep first. In October, there were only 20 cases, but this month the pace is even higher.
朝の緊急が終わって、少し睡眠を取り、予定手術の前立ちをしているとまた下行大動脈瘤切迫破裂の方が運ばれて来たのでこれから手術です。タイのジョー先生が僕が背部痛がすると言ったら背中をマッサージしてくれました。
After an emergency this morning and some sleep, I was the first assistant for a planned surgery. Another patient arrived with a threatened aortic aneurysm rupture, so I have another surgery. Dr. Joe from Thailand massaged my back when I said it hurt.
今A型解離の緊急手術が終わり、手術室から出るとまたA型解離の要請。さらにAAAR（腹部大動脈人工血管置換術）後の末梢側切迫破裂のEVAR（腹部大動脈瘤ステントグラフト内挿術）も同時並行で行います。病気は時間外に来ないわけではありません。来年から本格的に始まる時間外勤務制限の皺寄せは患者さんにいってしまいます。さらなる仕事効率化が必要です。
I just completed an emergency surgery for Type A aortic dissection and immediately received another similar case. We are also handling an EVAR for a post-AAAR impending rupture. With upcoming work-hour restrictions, optimizing efficiency is crucial for patient care.
歯石除去だけでもその数週間後に人工血管感染を起こした患者さんを今まで5人くらい見ました。歯石除去でも歯肉出血が多くなることがあるからだと思います。人工弁や人工血管がインプラントされている方は抜歯の時以外でも予防的抗生剤は飲んでから歯石除去をしてもらって下さい。というのが僕の意見です。
I’ve seen five patients develop graft infections after dental scaling, likely due to gum bleeding. My advice: those with artificial valves or grafts should take antibiotics not just for extractions, but also before scaling.
今月に入って急激に大動脈解離で運ばれてくる方が増えました。中には普段から健康に気を遣っている方も沢山います。50㍉の動脈瘤を指摘されている方は専門医に一度相談してください。また降圧薬の自己中断もせず、かかりつけに行きましょう。

川崎大動脈センターでは今朝まで緊急Bentall手術、その後胸腹部アプローチのEVAR後AAARを行い、緊急弓部置換が今行われております。そしてERにはB型解離が運ばれてきました。さらにA型解離とEVAR後仮性動脈瘤の患者が運ばれてきます。また冬が来たなという感じです。血圧管理には気をつけて下さい。
At the Kawasaki Aortic Center, we performed emergency Bentall surgery until this morning, then EVAR followed by AAAR using a thoracoabdominal approach, and emergency arch replacement is currently being performed. Type B aortic dissociation was then carried to the ER. In addition, a patient with a type A aortic dissection and a post-EVAR pseudoaneurysm is brought in. It feels like winter has come again. Please be careful about blood pressure control.
急に寒くなってきています。そのためか今日はA型解離が4件。2件は他の病院からの紹介でしたが、2件は直接当院の救急外来に来た方でした。状態が悪くてもオペ室に連れて行けば何とかなる。そう思える1日でした。一緒に頑張ってくれるER、技師、看護師、麻酔科、ICUスタッフに感謝です。
It’s suddenly getting cold. Perhaps that’s why there are 4 cases of type A acute aortic dissection today. Two cases were referrals from other hospitals, and two cases were patients who came directly to our emergency department. Even if the patient’s condition is bad, if you take him to the operating room, things will be fine. It was a day that made me think so. I would like to thank the ER, technicians, nurses, anesthesiology, and ICU staff for working hard together.
