朝6床空いていた大動脈センターのベッドが緊急入院が立て続き満床になりました。 凄い数の緊急患者が来ております。大動脈解離の患者数は波がありますが、経験的に年に患者が多くなる波は数回程度でした。しかし去年末から今年にかけて、この波が何度もあるので驚きです。
The six beds that were available in the morning are now full due to emergency. The patients with dissection comes in waves, but in my experience there have only been a few waves per year. But, from the end of last year, there have been several of these waves, which is surprising.
左開胸弓部下行人工血管置換術、直近3ヶ月7件、結果は良好。Tomasi先生によると合併症は緊急症例の出血再開胸1例。標準化出来れば治療の選択肢が広がります。左開胸弓部置換は当センターで69件。そろそろ標準化出来そうです。
Lt. thoracotomy TAR, 7 cases in the last 3 months, good results; Dr Tomasi said the complication was one case of bleeding in an emergency case. If it can be standardised, treatment options will be expanded. 69 cases have been performed at KAC. It’s about to be standardised.
本日A型解離の緊急搬送5件、B型解離破裂1件の搬送。手術を希望されなかった方もいます。現在も緊急手術中です。この数は寒波のせいでしょうか。破裂前に手術すればリスクは全然違います。既知の大動脈瘤がある方は一度専門医にかかってみてください。
Five emergency transports for type A and one ruptured type B today. They are still undergoing emergency surgery. Is this number due to the cold weather? The risk is totally different if you operate before rupture. If you have a known aortic aneurysm, please see a specialist.
11階の売店。クリスマスとお正月が終わり、もうバレンタインのシーズンに入りました。忙しい日々が毎日あっという間に過ぎてしまいます。SNSをやっていると、目標に向かうバイタリティ溢れる先生たちが多くいるので、僕ももっと頑張ろうと励まされます。毎日を目標を持って大切にしないといけません。
今日はKommerell’s diverticulum に対する右開胸下行大動脈人工血管置換術の後に川崎大動脈カンファレンスでした。今日はインドのVarun先生より左開胸弓部置換に関するトピックでRCP, ACPについてみんなで議論しました。学会と違いそれぞれが意見を言い合えるのでとても有意義な話ができました。
I joined the Kawasaki Aortic Conference after the surgery for Kommerell’s diverticulum today. Dr. Varun from India discussed RCP and ACP on the topic of left thoracotomy TAR. Unlike academic societies, we were able to exchange opinions, and have very meaningful discussions.
川崎大動脈センターの2022年は今までで一番忙しい一年になりました。今年は今のところ開胸による胸部大動脈手術574件、ステントグラフトはTEVAR, EVAR合わせて196件でした。どの患者さんにも最善の治療が出来るように来年も自分たちの技術を向上させるべく精進します。
KAC has had its busiest year to date in 2022. So far this year, we have performed 574 open thoracic aortic surgeries and 196 stent-grafts. We will continue to work hard to improve our skills next year so that we can provide the best possible treatment for every patient.
大動脈瘤をガイドラインに従って55ミリや60ミリまで待ってから治療介入となると中には待機中に破裂して亡くなってしまう方がいます。50ミリを超えた段階で専門施設での適切なフォローアップが必要です。大きさだけではなく拡大率や形、その方の年齢、体格などからも治療適応を考えます。
According to guidelines, waiting until 55 mm or 60 mm for an aortic aneurysm before intervention, some patients rupture and die while waiting. Appropriate follow-up at a specialized facility is required at the stage exceeding 50 mm. In addition to the size, we consider the indications for treatment based on factors such as the degree of enlargement, shape, age, and physique of the patient.
パン先生の送別会を行いました。6ヶ月の研修はあっという間に過ぎました。彼はこの6ヶ月で胸部大動脈手術130件に参加し多くを学んでくれました。川崎スピリッツも同時に学んでくれたと思います。タイでは多くの川崎ファミリーが頑張っていますが、彼のこれからの活躍を楽しみにしています。
We held a farewell party for Dr Pang, whose six months of training passed very quickly. He participated in 130 thoracic aortic operations in those six months and learnt a lot. I think he also learnt the Kawasaki spirit at the same time.
1日(24時間)に大動脈解離で運ばれてくる方が6件。A型4件、B型2件。川崎大動脈センターだけでこの数なので、全国的にも多い日なのでしょうか？気候が原因なのか、僕自身10余年大動脈治療に専念してきましたがこんなに多いのは初めてです。
In one day (24 hours) there were six emergency transfers to our centre for aortic dissection: four Type A and two Type B. Is this a day of high incidence of aortic dissection in Japan, since we are accepting so many admissions ourselves?
