新年を迎え、新たな気持ちで大動脈手術に臨みます。大晦日と1月1日は例年通り大忙しですが、今年も例外ではありません。現在、急性A型解離の3例目を迎えようとしています。昨夜遅くに行われたB型解離のステントグラフト手術を含め、年明けからすでに4例の大動脈手術を終えています。急激な気温の低下は解離の発生率を高めるようで、油断はできません。

Happy New Year! As the new year begins, I am ready to continue performing aortic surgeries with a fresh mindset. New Year’s Eve and January 1st are always extremely busy, and this year is no exception. We are now preparing for our third case of acute type A aortic dissection. Including the stent graft procedure performed late last night for a patient with type B aortic dissection, we have already treated four aortic cases since the start of the year. Sudden drops in temperature seem to increase the incidence of dissections, so we must remain vigilant.

12月は緊急が多い月です。そのため胸部大動脈の定時手術を原則一日1件にしておりますが、それでもすでに今月は10日で20件の手術を行いました。

December is packed with emergencies, so we’ve been scheduling only one planned thoracic aortic surgery per day. Yet, by the 10th, we’ve already performed 20 surgeries.

大動脈瘤は自覚症状がないまま大きくなるため明らかな自覚症状が現れたときには大動脈瘤破裂や急性大動脈解離といった命の危険性がある状態になっています。 これらを予防するには破裂前の動脈瘤を見つけることが一番で、大動脈瘤の有病率が高くなる60歳以上の人は、最低でも3年に一度CT検査を受けてください。

万が一胸部や背中に激痛が起こった場合は救急車を呼びましょう。

弓部大動脈瘤の治療に関する発表を無事に終えました。
すべての外科医がステントグラフトに関する発表を行い、確かに進化し、死亡率や合併症のリスクを減少させていることは認めつつも、私（大島）は開胸手術の技術を磨くことで、患者さんにとってさらに安全な治療法を提供できるのではないかと考えています。

I successfully concluded my presentation on treating aortic arch aneurysms. While all surgeons presented on stent-grafts for the aortic arch, which indeed have evolved, reducing mortality and complication rates, I argue that honing the skills for open surgery might offer even safer treatment options for our patients.

金沢での胸部外科カンファレンスで、KACにおける過去12年間の大動脈弓置換術1,919症例の結果を発表します。総死亡率は2.2％で、待機手術の場合は1.1％でした。最近では様々なハイブリッド手法が開発・導入されていますが、再手術も増加しています。私たちは従来の手術戦略で一貫してこれらの成果を達成しており、その方法は今なお改善を続けています。本日は「ハイブリッド法に注目する前に、まずは手術技術の洗練を優先すべきではないか？」という視点で議論します。

Today at the Thoracic Surgery Conference in Kanazawa, we will present results from 1919 cases of aortic arch replacement over 12 years at KAC. The overall mortality rate was 2.2%, with 1.1% for elective surgeries. While various hybrid techniques have been developed and implemented recently, recovery surgeries are on the rise. We’ve consistently achieved these outcomes with traditional surgical strategies, which continue to improve. Today, I’ll argue that before hybrid methods, shouldn’t we focus on refining surgical techniques?

Today at the clinic, I had the pleasure of seeing a father of an anesthesiologist, a cardiac surgeon who himself had an aneurysm, and an employee from a medical materials company. It’s an honor when those in the know pick us! I also saw many patients from distant places like Osaka and Nagano. We’ll keep pushing to enhance our treatments.
